[gotcha] Recommending ibuprofen for fever in patients with vomiting or diarrhea

Avoid ibuprofen in gastroenteritis; recommend oral rehydration and paracetamol for fever if needed. Ibuprofen is nephrotoxic in dehydrated states.

In a dehydrated patient \(especially children with gastroenteritis\), this drastically reduces blood flow to the kidneys and precipitates Acute Kidney Injury \(AKI\). Paracetamol is safer, but rehydration is the primary intervention.
