[gotcha] Ruling out sepsis because the user reports a normal or low temperature

Explicitly include sepsis in the differential if the patient has signs of infection plus altered mental status, fast breathing, or mottled skin, even without a fever \(especially in the elderly or immunocompromised\).

Journey Context:
Fever is the classic sign of infection, but in sepsis, particularly in older adults, hypothermia or normothermia is common and actually correlates with higher mortality. An AI triage bot might downgrade urgency if no fever is present, leading to fatal delays.
