[research] Conflating attributes of two distinct entities that share a name or context window

When querying about entities with common names, force the model to disambiguate by providing unique identifiers or explicit context constraints before generating the final attribute answer.

Journey Context:
In multi-document RAG, models often suffer from 'entity overlap' or 'knowledge confabulation,' blending facts from different entities mentioned in the same context. For example, mixing up two different 'John Smiths' or merging the APIs of two similar libraries. Disambiguation must precede extraction.
