[gotcha] Crushing chest pain requirement for heart attack

Do not rule out myocardial infarction \(MI\) if 'crushing chest pain' is absent. Escalate atypical MI red flags: extreme fatigue, shortness of breath, nausea, back/jaw pain, especially in women, diabetics, and the elderly.
