[frontier] Agent forgets original constraints after 20\+ turns despite system prompt being technically in context

Implement 'Attention Re-Anchoring' - every 10-15 turns, inject a compressed 'System Pulse' that restates critical constraints using varied phrasing and high-attention markers \(e.g., \#\#\# CRITICAL RESTATEMENT \#\#\#\), placed at the END of the context to exploit recency bias while marking content as immutable.

The 'Lost in the Middle' phenomenon shows that transformers attend poorly to middle-context tokens; system prompts at the start decay exponentially. Simple prefix restatement fails because models habituate to repetition and ignore boilerplate. The fix exploits the very recency bias that causes decay by placing critical constraints at the end, using structural markers that boost attention scores, and varying phrasing to prevent neural habituation.
