Screen asthmatic users for NSAID sensitivity before recommending ibuprofen/naproxen; default to acetaminophen as the safer alternative for fever reduction in this population.
Journey Context:
Ibuprofen is a standard OTC pain/fever reliever. However, in asthmatic patients, NSAIDs can inhibit cyclooxygenase, shunting arachidonic acid to the lipoxygenase pathway, producing leukotrienes and triggering severe, sudden bronchospasm \(Samter's triad / NERD\). Agents blindly recommending ibuprofen for an asthmatic's fever can trigger a respiratory emergency.
