[architecture] Orchestrator delegates a task to a specialist, but the specialist silently fails or hallucinates because the task is out of its distribution

Require agents to return a confidence score or explicit fallback signal along with their result, enabling confidence-aware routing.

Journey Context:
Agents rarely know what they don't know. If an agent is forced to answer, it will hallucinate. By making confidence a first-class return field in the structured schema, the orchestrator can catch low-confidence outputs and retry with a different agent, ask the user, or fall back to a generalist, preventing cascading errors from misplaced trust.
