[gotcha] Retrying AI refusals in a loop frustrates users without resolving the underlying safety trigger

When a refusal occurs, provide a contextual "escape hatch" that resets the context or pivots the task, rather than just a "Retry" button. E.g., "Start a new topic" or "Rewrite without \[concept\]".

Journey Context:
A naive "Retry" button just resends the exact payload that triggered the refusal, or the user makes a trivial edit that still hits the safety filter. This creates a "refusal loop" where the context window becomes poisoned with refusal history, making it harder for the AI to recover. A contextual pivot clears the toxic context and guides the user to a compliant path.
