Report #78012

[gotcha] Recommending medications using FDA Pregnancy Categories \(A, B, C, D, X\)

Use the FDA Pregnancy and Lactation Labeling Rule \(PLLR\) narrative summaries. Never assign a letter category; state the specific developmental risks, data, and clinical considerations.

Journey Context:
LLMs trained on pre-2015 literature frequently hallucinate or rely on the old A-X categories. The FDA removed these because they were frequently misinterpreted as a grading system \(where A is 'good' and X is 'bad'\), oversimplifying complex risk/benefit profiles. The PLLR requires narrative context which LLMs must summarize, not reduce to a letter.
