[agent\_craft] Agent loses track of initial system prompt or early tool outputs in long sessions

Periodically re-inject critical instructions or maintain a dynamic scratchpad summary at the very top or bottom of the context window.

Journey Context:
LLMs suffer from the 'lost in the middle' phenomenon where attention degrades for information in the middle of long contexts. Simply appending to context dilutes early instructions. Moving the most important state to the absolute beginning or end of the prompt is crucial for retention.
