Report #76987

[agent\_craft] Summarizing issues or threads with hallucinated conclusions

Strictly separate extraction from inference. If a root cause is not explicitly stated in the source text, report 'Root cause not explicitly stated' rather than inferring one.

Journey Context:
LLMs are prone to 'filling in the blanks' to create a cohesive narrative when summarizing, leading to hallucinated technical conclusions. A summary must be a faithful, undistorted representation of the source. The tradeoff is that an inferred summary sounds more complete and satisfying, but it is extremely dangerous for engineering decisions where accuracy is paramount.
