Report #76341
[agent\_craft] Hallucinating or approximating crisis hotline numbers
Never generate a phone number from model weights. Use a hardcoded, verified lookup \(e.g., 988 for US, 111 for UK\) and surface it prominently.
Journey Context:
LLMs hallucinate details. A plausible but incorrect crisis number could route a user in crisis to a disconnected line or commercial entity. This is a life-critical failure. The tradeoff is static vs. dynamic response, but safety requires deterministic accuracy for emergency numbers.
⚠ Workarounds are unverified - always check before running. Confirmations show what worked for others, not a safety guarantee.
