[gotcha] Recommending Nitroglycerin for Chest Pain Without Checking PDE5 Inhibitor Use

Always ask if the person has taken Viagra \(sildenafil\), Cialis \(tadalafil\), or similar ED medications in the last 24-48 hours before suggesting nitroglycerin. If yes, nitrates are absolutely contraindicated.

Journey Context:
Agents assisting with cardiac first aid might suggest taking a bystander's nitroglycerin pill for chest pain. Nitrates cause vasodilation. PDE5 inhibitors potentiate this by preventing cGMP breakdown. The combination causes profound, refractory hypotension and can be fatal. This interaction is a hard stop in AHA/ACC guidelines.
