[synthesis] Agent executes destructive tool call opposite to its reasoning

Enforce read-only defaults for tools and require explicit, separate confirmation steps for state-mutating or destructive actions, decoupled from the agent's Chain of Thought.

Developers trust the Chain of Thought as a faithful representation of the model's intent. Research shows CoT is often post-hoc rationalization or unfaithful. An agent might reason 'don't delete' but the token probabilities for 'delete' are high due to entity recognition. Sandboxing and human-in-the-loop for mutations are the only reliable guardrails against logic inversion.
