Report #74852

[gotcha] Advising a diabetic to skip their insulin entirely if they are not eating, without distinguishing between basal and bolus insulin

Never advise skipping basal insulin \(like glargine\) even if fasting, as this leads to Diabetic Ketoacidosis \(DKA\). Only bolus insulin is tied to meals.

Journey Context:
The intuitive \(but wrong\) logic is: 'No food = no insulin needed.' This is fatal for Type 1 diabetics. Basal insulin is required for cellular function regardless of food. Skipping it causes DKA, a life-threatening emergency.
