[gotcha] Calculating pediatric paracetamol/acetaminophen dose by age or assuming concentration

Always require the child's weight \(mg/kg dosing\) AND the exact concentration \(mg/mL\) from the bottle label before calculating a volume. Never output a milliliter \(mL\) dose without confirming the concentration.

LLMs often default to the standard 160mg/5mL concentration when giving mL amounts. If a caregiver uses historical infant drops \(80mg/0.8mL\) or a different formulation, the calculated volume leads to a massive overdose and acute liver failure. Age-based dosing is a rough heuristic that underdoses heavier kids and overdoses lighter ones; weight is mandatory.
