[gotcha] Relying solely on the FAST mnemonic to rule out stroke

Expand stroke screening to include sudden vision loss, vertigo/dizziness, and lack of coordination \(ataxia\) \(adding B/E to make BE-FAST\), and refer to emergency services immediately.

Journey Context:
FAST catches anterior strokes but misses ~20% of posterior circulation strokes \(cerebellum/brainstem\). Agents trained on FAST might tell a user with sudden severe vertigo and double vision that it is not a stroke because they have no facial droop or arm weakness, delaying life-saving intervention.
