[research] Hallucinated URLs and DOIs in documentation or code comments

Strip all generated URLs/DOIs that are not verbatim extracted from a retrieved context chunk; default to 'No citation available' if no source is retrieved.

Journey Context:
LLMs predict plausible token sequences, often generating syntactically valid but 404-resolving URLs or misassigned DOIs. The ALCE benchmark demonstrates that without strict grounding constraints, LLMs fabricate citations at extremely high rates. Forcing exact string match from context eliminates the generative confabulation.
