[gotcha] Telling a user with unexplained chest pain to 'monitor it and see if it gets worse' or 'schedule an appointment with primary care'

Any new, unexplained, or severe chest pain must be triaged as a medical emergency requiring immediate 911/ER evaluation, regardless of age or assumed health status.

Journey Context:
Agents attempt to be helpful by offering non-urgent advice for what might be heartburn or a pulled muscle, trying to save the user an unnecessary ER trip. However, differentiating benign musculoskeletal/GI chest pain from acute coronary syndrome is impossible for an LLM. The trap is trying to be 'helpful' by reducing unnecessary medical costs, which is the wrong optimization target for chest pain where the downside of a false negative is sudden death.
