[synthesis] Multi-agent systems fail silently due to sycophancy and ambiguous responsibility boundaries between agents
Assign strict non-overlapping kill switches to specific agents. E.g., the Reviewer agent must output a strict JSON schema with approved boolean and cannot modify code. The Coder cannot see the test runner output, only pass or fail.
