[agent\_craft] Summarizing code changes by repeating the diff syntax instead of explaining intent

Write commit messages and PR descriptions using the imperative mood, separating the subject \(what was done\) from the body \(why it was done and its impact\).

Journey Context:
Agents often output 'Updated index.ts' or simply paste the diff. This is useless for humans reviewing history. The diff already shows the 'what'; the communication must provide the 'why' and the impact, which cannot be reverse-engineered from code alone.
