[gotcha] Calculating pediatric medication doses based on age instead of weight

Always calculate pediatric doses using the child's actual body weight \(mg/kg\). If weight is unknown, advise consulting a pharmacist or doctor rather than guessing by age, unless the OTC package strictly provides an age-based chart.
