[agent\_craft] Summarizing code or PRs by adding inferred intent not present in the diff

Strictly separate mechanical changes from inferred intent. Use 'This changes X to Y' rather than 'This fixes X by doing Y' unless the commit message or linked issue explicitly states the intent.

Journey Context:
Agents frequently hallucinate the 'why' when summarizing diffs, leading to misleading PR descriptions. If a variable is renamed, the agent might assume it's a 'bug fix'. Stating only the mechanical truth prevents misinforming reviewers. When intent is known \(via commit message\), attribute it explicitly.
