[frontier] MCP tool servers blowing up agent context windows with massive schemas

Implement MCP Context Throttling: use a lightweight registry tool to list capabilities, then dynamically load the specific tool schema only when invoked, rather than injecting all schemas into the system prompt.

Journey Context:
Naive MCP integration exposes all tool schemas in the system prompt. As servers scale to dozens of tools, context fills up, degrading reasoning and increasing cost. The shift is from static schema injection to dynamic, on-demand schema resolution, treating MCP as a lazy-loaded namespace.
