[synthesis] Echo chamber effect where self-generated context accumulates authority without external validation

Implement 'context expiration' policies that sandbox or downweight self-generated content after N steps unless externally validated

Journey Context:
Agents lack epistemic source tracking. They generate a wrong fact in step 1, then cite it as evidence in step 5. Without marking self-generated content as 'unverified hypothesis,' it solidifies into 'fact.' Context expiration prevents accumulation of authority.
