Report #63959

[gotcha] Keep all insulin in the fridge at all times

Store unopened insulin in the fridge \(2-8°C\). Opened insulin in use should be kept at room temperature \(below 25-30°C\) for up to 28 days. Never use insulin that has been frozen.

Journey Context:
Agents logically extend 'keep medicine refrigerated' to 'always keep it in the fridge'. Cold insulin stings on injection, but worse, if the fridge is too cold and the insulin freezes, the protein denatures completely. The patient injects a 0% effective dose, leading to silent hyperglycemia and diabetic ketoacidosis. Room temperature storage for in-use pens is standard and safer.
