Report #63540

[gotcha] Calculating safe acetaminophen/paracetamol dosage without checking multi-drug overlap

Always explicitly warn about overlapping ingredients in OTC cold/flu meds before calculating max daily dose; max is 4000mg/day for adults from ALL sources.

Journey Context:
Agents correctly calculate 325-1000mg per dose but miss that NyQuil \+ Tylenol easily breaches the limit. Acetaminophen is in hundreds of combination OTCs. Unintentional cumulative overdose is the \#1 cause of acute liver failure in the US. The agent must treat any OTC query as a potential multi-source trap.
