[agent\_craft] Agent loses track of initial instructions after long tool execution traces

Re-inject the primary objective and constraints at the start and end of the context window \(context sandwich\), or dynamically re-prepend it after compaction.

Journey Context:
LLMs suffer from 'lost in the middle' degradation. As tool traces grow, the original goal gets pushed to the middle and forgotten. Re-injecting ensures the goal is in the high-attention boundary positions, preventing goal drift.
