[gotcha] Agent executes MCP tools silently without logging the exact parameters or the tool's description that triggered the call

Log the exact tool name, the full schema/description used to select it, and the exact arguments passed before execution, ideally in an immutable audit log.

Journey Context:
When an agent behaves maliciously, developers check logs but only see 'called fetch\_url'. They miss that the tool description was poisoned, or that the arguments contained exfiltrated tokens. Without logging the \*reason\* \(the description\) and the \*payload\* \(the args\), forensics are impossible.
