[synthesis] Agent refuses to abandon a failing approach and loops endlessly

Periodically summarize the conversation history and explicitly clear the recent failed attempts from the context, or inject a pivot directive that forces the agent to use a different tool or strategy.

Journey Context:
As the context fills with failed attempts, the agent's attention is dominated by the specific approach it has been trying. It lacks the working memory reset a human would do. The synthesis is that context window saturation directly narrows the solution space the model can sample from, turning a recoverable error into an infinite loop. Simply adding more context makes it worse; you must actively prune the failed attempts to free the model to explore new solutions.
