[architecture] Old memories polluting current context window

Implement a two-phase retrieval: semantic search followed by temporal decay scoring, and only inject memories with a relevance score above a dynamic threshold into the active context.

Journey Context:
Agents often dump all semantically similar memories into the prompt. This causes distraction and hallucination. Alternatives: strict time-to-live \(too rigid\). Tradeoff: dynamic threshold might miss distant but crucial memories, but protects the context window from noise.
