[synthesis] Agent guesses user intent and takes wrong action instead of asking for clarification

Explicitly add a \`ask\_user\_for\_clarification\` tool and instruct the agent in the system prompt that using this tool is a valid and preferred action when intent is ambiguous.

Journey Context:
LLMs are fine-tuned to be 'helpful', which they interpret as 'take action'. Inaction is implicitly penalized. This leads to agents building complex solutions on flawed foundations rather than asking a simple question. By making 'asking' a first-class tool, you remove the friction and stigma of inaction, allowing the agent to safely resolve ambiguity.
