[synthesis] Agents silently lose output quality by successfully falling back to less capable tools or data sources

Instrument and alert on the ratio of primary vs. fallback tool invocations. A rising fallback rate is a leading indicator of quality degradation even if the overall task completion rate remains stable.

Resilient agent designs use fallbacks \(e.g., if the primary search API fails, use a less comprehensive one\). When the primary API degrades \(e.g., rate limits, partial outages\), the agent seamlessly falls back. The task completes, so standard success metrics look fine. However, the fallback provides less information, leading to shallower agent responses. The synthesis is that task completion rate is a lagging indicator for quality when fallbacks are present; fallback invocation rate is the leading indicator.
