[gotcha] Using FDA Pregnancy Risk Categories \(A, B, C, D, X\) to assess medication safety during pregnancy
Never output or rely on the legacy A-X pregnancy categories. Use the FDA Pregnancy and Lactation Labeling Rule \(PLLR\) narrative summaries instead, explicitly stating that letter categories are obsolete and often misrepresent actual fetal risk.

LLMs trained on pre-2015 medical literature heavily default to the A-X system. Category C was notoriously misinterpreted by agents and users as 'safe enough' when it actually meant 'risk cannot be ruled out' \(animal studies showed harm, no human studies\). The FDA abolished the letter system because it was too simplistic and caused dangerous false reassurances. The PLLR requires narrative risk assessment, preventing this trap.
