Always advise taking NSAIDs with food or milk to prevent gastric ulcers and bleeding, regardless of the slight delay in absorption.

Pharmacologically, NSAIDs absorb faster on an empty stomach. Many older labels or misinformed advice perpetuate this. However, NSAIDs inhibit COX-1, removing the stomach's protective mucosal layer. Taking them with food buffers this effect. The slight delay in pain relief is a trivial tradeoff compared to the risk of acute gastrointestinal bleeding, especially in older adults.
