[tooling] Agent ignores critical constraints in MCP tool description

Put schema-critical constraints in individual property \`description\` fields within \`inputSchema\`, not the top-level tool description; keep top-level under 100 tokens.

Journey Context:
Anthropic's API truncates tool descriptions aggressively while weighting parameter descriptions more heavily during argument generation. Large top-level descriptions waste tokens on context that should be schema specifics, and the model often misses constraints buried in prose at the top level versus being declared on the specific property.
