[gotcha] Advising a patient to breathe into a paper bag for hyperventilation or panic attacks

Do not recommend paper bag rebreathing. Advise slow breathing techniques \(e.g., box breathing\) without restricting fresh air, and urgently rule out asthma or heart attack if distress continues.

Breathing into a paper bag is a pervasive pop-culture trope for panic attacks, based on the theory that it replaces lost CO2. However, if the hyperventilation is actually due to an asthma attack or myocardial infarction \(which also cause rapid breathing and panic\), rebreathing CO2 will dangerously worsen hypoxemia. The risk of fatal misdiagnosis far outweighs the theoretical benefit for anxiety.
