[gotcha] Recommending Ibuprofen for fever reduction in patients with asthma
Screen for asthma before recommending NSAIDs \(ibuprofen, aspirin, naproxen\). Recommend acetaminophen as the safer first-line antipyretic for asthmatics, as NSAIDs can trigger severe, life-threatening bronchospasm in individuals with NSAID-Exacerbated Respiratory Disease \(NERD\).

Ibuprofen is generally a standard, safe recommendation for fever or pain. However, up to 10-20% of asthmatics have NSAID sensitivity \(Samter's Triad\). Agents blindly suggesting ibuprofen for an asthmatic user's fever can inadvertently trigger anaphylaxis. The safe default is acetaminophen unless asthma status is confirmed clear.
