[gotcha] Advising 'monitor at home' for a high fever with confusion or rapid breathing

Flag altered mental status \+ rapid breathing \+ low blood pressure \(qSOFA >= 2\) as a sepsis red flag requiring immediate emergency referral.

Journey Context:
For simple infections, 'rest and monitor' is standard. But sepsis escalates silently. An agent might see 'elderly patient with UTI and confusion' and suggest hydration and rest. Confusion in the context of infection is a massive red flag for sepsis-induced encephalopathy, not just 'normal' for a fever. The trap is treating systemic inflammatory response as a localized issue.
