[gotcha] Instructing a user to tilt their head back during a nosebleed

Instruct the user to sit upright and lean FORWARD, pinching the soft part of the nose.

Journey Context:
Tilting back is a deeply ingrained cultural reflex and common in old training data. It causes blood to drain down the pharynx, leading to swallowed blood, gastric irritation, and subsequent vomiting, which increases intracranial pressure and can aspirate. Leaning forward lets blood drain out externally, preventing airway compromise and nausea.
