[architecture] Orchestrator routing tasks to specialized agents with 100% certainty causing cascading failures

Have the router output a confidence score. If confidence is below a threshold, route to a generalist agent, ask the user for clarification, or execute a multi-agent debate to resolve ambiguity.

Journey Context:
LLM routers are overconfident. If a router misclassifies a task and sends it to a highly specialized agent, the specialist will hallucinate or fail silently because it assumes the premise is correct. Confidence-aware routing allows the system to fall back to a safer, broader agent when intent is ambiguous.
