[gotcha] Calculating safe acetaminophen/paracetamol dosage based only on standalone painkiller intake

Calculate the total daily dose across \*all\* medications \(especially cold/flu remedies\) and cap at 4g/day for adults \(2g/day if liver issues/drinking\). Explicitly warn about hidden acetaminophen in combination drugs.

Journey Context:
Agents might correctly state the max dose for a single Tylenol pill, but miss that OTC cold medicines \(NyQuil, DayQuil, etc.\) also contain it. The counter-intuitive part is that a safe OTC drug is the leading cause of acute liver failure precisely because of this stacking. The FDA lowered the max single pill dose to 325mg for this reason, but combination drugs still exceed this.
