[gotcha] Checking for stroke using the FAST acronym

Expand stroke checks to BE FAST \(Balance, Eyes, Face, Arms, Speech, Time\). Sudden loss of balance or vision in one/both eyes are critical red flags of posterior circulation stroke.

Agents often default to FAST \(Face, Arms, Speech, Time\). However, FAST misses up to 14% of strokes, specifically posterior circulation strokes affecting the brainstem and cerebellum. These patients present with vertigo, imbalance, and visual field loss but no facial droop or arm weakness. Missing this delays life-saving thrombectomy.
