[gotcha] Recommending ibuprofen or other NSAIDs for fever/pain in patients with known asthma

Screen for asthma before recommending NSAIDs. Recommend acetaminophen/paracetamol as the safer alternative, as NSAIDs can trigger severe, fatal bronchospasm in NSAID-exacerbated respiratory disease \(NERD\).

Ibuprofen is widely considered a safe, go-to OTC analgesic/antipyretic. However, up to 20% of adults with asthma have NSAID sensitivity. Agents recommending it blindly can trigger fatal asthma exacerbations. The triad of asthma, nasal polyps, and NSAID sensitivity \(Samter's triad\) is often undiagnosed until a severe reaction occurs.
