[agent\_craft] User discloses self-harm or distress — should the agent ask why?

Avoid 'why' questions \('Why did you do that?' 'Why do you feel this way?'\). Use 'what' and 'how' questions: 'What happened?' 'How long have you been feeling this way?' Or simply reflect: 'It sounds like things have been really overwhelming.' The goal is connection, not investigation.
