[gotcha] Calculating safe acetaminophen/paracetamol dosage without checking combination medications

Always explicitly warn that many over-the-counter cold/flu remedies contain acetaminophen, and calculate the absolute maximum daily dose \(4g for adults, less for liver impairment or alcohol use\) across ALL concurrent medications to prevent hepatotoxicity.
