Report #46977

[counterintuitive] AI code review catches all bugs humans catch plus syntax errors

Use AI strictly for local invariant checking and style; mandate human review for state transitions, API boundaries, and implicit business rules.

Journey Context:
Humans are bad at syntax and get tired, making AI seem superior. However, AI evaluates code locally and suffers from attention dilution in long contexts, missing architectural drift and implicit global state. Humans implicitly hold the system's threat model and state machine, catching bugs AI fundamentally cannot see.
