[frontier] Context Anchor Drift: System instruction decay in long-context windows

Implement a Periodic System Refresh protocol that re-injects the core identity and constraints every N turns \(e.g., every 10-20 exchanges\) or at entropy thresholds, using a fresh context insertion rather than appending to history.

Journey Context:
Teams often assume longer context windows solve drift, but this exacerbates 'Lost in the Middle' attention decay. Simple truncation loses critical constraints. The refresh must occur \*before\* drift manifests, not as a reactive fix. This trades token cost for consistency, but is cheaper than post-hoc correction of a runaway agent.
