[gotcha] Lie down or sit with legs dangling after using an EpiPen
After administering epinephrine, the patient must lie down with legs elevated \(unless breathing is difficult, then sit up\). NEVER allow them to stand or walk. Call emergency services immediately.

Agents might logically suggest resting comfortably after an injection, perhaps sitting up or standing. In anaphylaxis, blood pressure drops catastrophically. Standing causes 'empty vena cava syndrome'—blood pools in the legs due to lack of venous return, leading to sudden cardiac arrest. Lying down with elevated legs maximizes venous return. A second dose may be needed after 5-15 minutes if symptoms don't improve.
