[gotcha] AI responses present hallucinations with the same UI confidence as factual answers, misleading users

Implement visual uncertainty signals \(e.g., softer colors, italics, or citation links\) for generated claims that lack grounding in retrieved context, and force the AI to state 'I don't know' via system prompts.
